Many people who ask us about implants have already been told by a US dentist that they need a bone graft first. Few knew what that meant, only that the quote had grown by $1,000 to $3,000 and the finish date had moved back by half a year. Here is who needs one, what it costs, and how it changes the timeline.
A bone graft is needed when the jaw does not have enough bone to hold an implant, and a CBCT scan decides that, not a guess. In the US a socket graft costs about $300 to $800 and a sinus lift $1,000 to $3,500 per side, on top of the implant. Most grafts need 3 to 6 months to become bone, so the process takes 8 to 12 months instead of 4 to 6. Tilted implants (All-on-4) or short implants often avoid a graft. In Tbilisi, bone graft and sinus lift are not in the fixed package; they are quoted in writing before you travel, from your scan.

ToggleWhat a bone graft for a dental implant is
A dental bone graft is bone, or a bone-like material, placed where your jaw is too thin or too short to hold an implant. Your body grows new bone through it over several months, then the implant goes in.
An implant needs about 1.5 to 2 mm of bone around it and enough height to clear the sinus or the nerve. Graft material is your own bone (autograft), donor bone (allograft), bovine bone (xenograft) or a synthetic mineral (alloplast). A 2014 meta-analysis found implant survival did not depend on the material, so most clinics now use donor or bovine granules.
The four graft types, healing time and US cost
The four common procedures are socket preservation, ridge augmentation, sinus lift and block graft. Socket preservation is the smallest and often done the day a tooth is pulled. A block graft is the largest.
US prices are from Real Dental Costs (August 2026) and KYT Dental Services, a California clinic (June 2026).
| Graft type | Used when | Healing before the implant | Typical US cost per site |
|---|---|---|---|
| Socket preservation | Same day as extraction | 3 to 4 months | $300 to $800 |
| Ridge augmentation | Ridge too thin or low | 4 to 6 months | $800 to $2,000 |
| Sinus lift, per side | Upper back jaw, bone too short | Internal: same day as implant. Lateral: 4 to 9 months | $1,000 to $1,800 internal; $1,500 to $3,500 lateral |
| Block graft | Large defect | 4 to 6 months or longer | $2,000 to $3,000 or more |
Who needs a bone graft
A CBCT scan decides it, not the dentist’s eye. The scan measures bone height and width in millimetres where the implant will go. If the numbers are there, no graft.
Grafts are most common in the upper back jaw, where bone under the sinus often falls below 5 mm, at a front tooth lost years ago, and where a tooth was lost to infection. Recent extractions, lower molars and full-arch cases usually need none. If a graft is recommended from a 2D X-ray alone, ask for a CBCT.
How often the graft can be avoided
For a full arch, a graft can usually be avoided; for a single upper back tooth, it often can be. The tools are tilted implants, the basis of All-on-4, and short implants of 5 to 8 mm. In All-on-4 the two back implants are angled up to 45 degrees to anchor in solid bone in front of the sinus or the nerve. A 2015 meta-analysis of 44 studies and 10,761 implants found no significant difference in failure between tilted and axial implants (1.63 against 1.81 per cent). Maló and colleagues reported 96.4 per cent implant survival at 3 years for trans-sinus tilted implants placed with no sinus graft. For a single molar under a low sinus, a 2018 meta-analysis of 11 randomised trials found the same survival for short implants as for longer implants with a sinus lift.
Sinus lift: what it is and what it costs
A sinus lift adds bone height under the sinus in the upper back jaw. The internal lift goes through the implant hole and is usually done with the implant, when 5 to 8 mm of bone remain. The lateral window lift opens the side wall, packs in graft and waits months, and is the norm below 4 to 5 mm.
Real Dental Costs puts the internal lift at $1,000 to $1,800 per side and the lateral window at $1,500 to $3,500, plus $400 to $1,200 for material and membrane (May 2026). Pearl’s national data gives $1,470 to $3,860. A 2008 review of 12,020 implants found up to 98.3 per cent 3-year survival with rough-surface implants and a membrane. A 2024 meta-analysis found 97.1 per cent survival even when the membrane tore and was repaired.
Bone graft cost in the US, item by item
A graft adds a few hundred to several thousand dollars to an implant. The national average single implant is $4,500, from $3,759 in Alabama to $5,733 in California (Dental Implant Cost By State, 2026). The graft is billed on top.
Real Dental Costs lists donor or bovine material at $300 to $1,800 and your own bone at $2,000 to $5,148. Pearl’s ridge graft figure (D7953) is $680 to $1,790, average $1,045. A CBCT is $100 to $250 and IV sedation $400 to $1,500. Insurance rarely helps: grafts for an implant are often excluded, or paid at 40 to 50 per cent up to a $1,000 to $2,000 annual cap. See our guide to insurance and implants.
How a graft changes the timeline
Without a graft, a single implant takes about 4 to 6 months from consultation to crown. With a staged graft it takes 8 to 12 months, because the graft must become bone before the implant is placed, then the implant must fuse before the crown goes on.
A 2019 meta-analysis found most trials placed the implant 4 months after a socket graft, with 95 to 100 per cent implant survival. Ridge and lateral sinus grafts need 4 to 6 months. The surgeon confirms on a second CBCT, not on the calendar.
| Stage | Without a graft | With a staged graft |
|---|---|---|
| Consultation and CBCT | Week 0 | Week 0 |
| Graft surgery and healing | Not needed | Week 1, then 3 to 6 months |
| Implant placed | Week 1 to 2 | Month 4 to 7 |
| Implant fuses | 3 to 6 months | 3 to 6 months |
| Final crown | Month 4 to 6 | Month 8 to 12 |
| Trips to Tbilisi | One stay of about 10 days | Two short stays |
What a graft means for a trip to Tbilisi
If your scan shows a graft is needed, we tell you before you book, and the plan changes from one visit to two. The graft is done at the first visit, you heal at home, and you return for the implants. The written quote lists both trips.
If no graft is needed, a single implant is about 10 days in Tbilisi, and the crown is fitted once the implant has healed. Your written plan says whether that happens within the stay or at a second short visit, and what is included. Most full-arch cases avoid the graft with tilted implants, which is one reason All-on-4 in Tbilisi is $4,500 an arch. Bone graft, sinus lift and sedation are extras, priced in writing before you travel; we do not publish a graft price because it depends on what your scan calls for. One honest note: one implant with a staged graft means two trips, which rarely beats a US surgeon who sees you at every stage. Savings become real at two or more implants, or a full arch.

Pain after a graft compared with a plain implant
A plain implant usually hurts less than an extraction. A graft hurts a little more and for a little longer, mostly as swelling and tightness. Lateral sinus lifts and block grafts may need prescription painkillers for two or three days.
Soreness runs 3 to 5 days after a socket graft and 3 to 7 days after a ridge, sinus or block graft, with 2 to 4 days off work. Pain that worsens after day three, a bad taste, pus or fever are not normal. Call the clinic.
Aftercare after a bone graft
The graft is fragile for two weeks. Do not touch it, do not suck on it, do not smoke, and keep the mouth clean without brushing the site. Most early failures come from infection or a disturbed membrane.
- Days 1 to 3: cold packs, head raised in bed, soft cool food, no straws, no spitting. Finish any antibiotics.
- Days 3 to 14: warm salt-water rinses after meals, chew on the other side. Losing a few grains is normal.
- Sinus lift: no nose blowing, sneeze with the mouth open, no flying or heavy lifting for about 2 weeks.
- Smoking: the biggest single risk. Stop 2 weeks before and 8 weeks after.
Signs a graft or implant is failing
Graft failure shows early, in the first 3 to 4 weeks, as an exposed membrane, graft washing out, or infection. Implant failure shows later, as an implant that moves, hurts on biting, or sits in gum that bleeds and recedes. Both are uncommon. The 2009 ITI review by Jensen and Terheyden found survival in grafted bone comparable to implants in native bone.
Frequently asked questions
Is a bone graft always necessary for a dental implant?
No. Most lower jaw implants and recent extractions need none. Upper back teeth and long-missing front teeth need one more often. Only a CBCT scan can say. For a full arch, tilted implants usually avoid grafting, so most All-on-4 cases are planned without one.
How much does a dental bone graft cost in the US?
About $300 to $800 for a socket graft, $800 to $2,000 for ridge augmentation, $1,000 to $3,500 per side for a sinus lift, and $2,000 to $3,000 or more for a block graft, on top of a $3,000 to $6,500 implant.
How long after a bone graft can I get the implant?
Usually 3 to 4 months after a socket graft and 4 to 6 months after a ridge graft or lateral sinus lift, with xenograft at the longer end. The surgeon confirms with a second CBCT. Internal sinus lifts are often done the same day as the implant.
Does a bone graft hurt more than an implant?
A little. A plain implant is usually milder than an extraction. A graft adds swelling and tightness for 3 to 7 days, and a lateral sinus lift or block graft may need prescription painkillers for two or three days. Using your own bone adds a second sore site.
What are the signs a dental implant is failing?
An implant that moves, pain on biting after the crown is on, gum that bleeds or recedes around it, swelling or pus, and bone loss on an X-ray. Early failure is usually a fusion problem; later failure is usually peri-implantitis, a gum infection around the implant.
Does the Tbilisi price include a bone graft or sinus lift?
No. The fixed package covers the scan, surgical guide, implants, surgery, final crown or bridge, transfer and coordinator. Bone graft, sinus lift and sedation are extras, quoted in writing from your CBCT before you book, so the total and the number of visits are known before you buy a ticket.
The short version
- A CBCT scan decides whether you need a graft. Upper back teeth need one most often; full-arch cases on tilted implants usually do not.
- In the US a graft adds $300 to $3,500 per site and 3 to 6 months of healing; a single implant then takes 8 to 12 months.
- In Tbilisi grafts and sinus lifts are extras quoted in writing before travel; implant and arch prices are fixed at $730, $1,450 and $4,500.
