Dental implants and German health insurance: what AOK, TK and Barmer really pay. Festzuschuss, top-up insurance and your share in 2026

German statutory insurance pays the fixed subsidy (Festzuschuss) for a bridge or denture, €552.96 to €691.20 for a single gap in 2026. What AOK, TK and Barmer cover, what top-up insurance adds, who gets hardship rates, why Tbilisi stays cheaper.

Heil- und Kostenplan, Versichertenkarte und Bonusheft auf einem Schreibtisch

A dental implant with a crown costs €2,100 to €4,500 in Germany. Many insured patients hope that AOK, TK or Barmer, the large German statutory health insurers, will cover a large part of it. They do not. The insurer pays exactly one amount, and that amount has nothing to do with the implant. Here are the fixed subsidies for 2026, three worked examples, top-up insurance, the hardship rule, and why Tbilisi stays cheaper even without any subsidy.

German statutory health insurance (gesetzliche Krankenversicherung, GKV) never pays for the implant itself. It pays the diagnosis-based fixed subsidy (Festzuschuss) that it would also pay for a bridge or a denture: 60% of the standard treatment (Regelversorgung), 70 or 75% with a complete dental check-up booklet (Bonusheft), 100% under the hardship rule (Härtefall). For a single missing tooth that is between €552.96 and €691.20 in 2026; the rest is your own share. This is the same with every insurer.

1. Does German health insurance pay for dental implants? The short answer

No, not the implant. Under Section 55 of the Social Code Book V (§ 55 SGB V), the insurer pays a fixed subsidy towards the standard treatment, meaning the simple bridge or denture for your diagnosis. An implant counts as a different type of treatment (andersartige Versorgung): same subsidy, but the treatment is billed privately under the German private fee schedule for dentists (GOZ).

Narrow exceptions are set out in § 28 (2) SGB V together with the treatment guideline of the Federal Joint Committee (G-BA): large jaw or facial defects after a tumour, accident or malformation, extreme dry mouth after tumour treatment, genetic absence of most teeth in one jaw, uncontrollable muscle disorders in the mouth area, and in each case only if treatment without implants is not possible. None of these applies to an ordinary gap.

2. Fixed subsidy 2026: the amounts that are actually paid

New fixed subsidies have applied since 1 January 2026. For a single missing tooth (diagnosis code 2.1) every insurer pays €552.96 without bonus, €645.12 after five and €691.20 after ten years of a complete check-up booklet. This is confirmed by the Verbraucherzentrale, the German consumer advice centre (as of 26 January 2026).

The diagnosis decides, not your choice. For an edentulous jaw the standard treatment is the full denture, even with All-on-4. The amounts in the table come from the fixed subsidy table 2026 (as of October 2026). The decision of the G-BA remains the binding source.

Diagnosis (standard treatment) 60% 70% 75% Hardship 100%
2.1 Single missing tooth (bridge) €552.96 €645.12 €691.20 €921.60
3.1 Free-end gap, per jaw (partial denture) €571.29 €666.51 €714.11 €952.15
4.2 Edentulous upper jaw (full denture) €576.44 €672.52 €720.56 €960.73
4.4 Edentulous lower jaw (full denture) €617.72 €720.68 €772.16 €1,029.53

Anyone who waits until 2027 to be treated gets less. The GKV Contribution Rate Stabilisation Act (GKV-Beitragssatzstabilisierungsgesetz), passed on 10 July 2026, lowers the rates to 50, 60 and 65% from 1 January 2027, according to the Paritätische Gesamtverband, a German welfare umbrella association (13 July 2026). A treatment and cost plan (Heil- und Kostenplan) approved in 2026 keeps its rates.

3. AOK, TK, Barmer, DAK: are there any differences?

Not in the fixed subsidy. The amount is set by law and guideline; no insurer may pay more or less. Anyone searching for “dental implant cost AOK” gets the same answer as with TK or Barmer: €552.96 for a single missing tooth without bonus.

Differences exist only around the edges: bonus programmes for check-up appointments, optional tariffs and brokered dental top-up insurance (Zahnzusatzversicherung), meaning private contracts with their own waiting period. Ask your insurer: is there a bonus programme, does my check-up booklet count as complete, and will the insurer check my treatment and cost plan free of charge.

4. Worked example: single implant, All-on-4, edentulous jaw

For a single implant in Germany, your own share after the fixed subsidy is around €2,300 to €2,450. For an edentulous jaw with All-on-4 it is around €14,400, because the denture subsidy hardly counts against €15,000. The figures are an example, not a quote.

German prices: the middle of the usual range, so €3,000 for a single implant with crown, €15,000 for one All-on-4 arch, €30,000 for both jaws, fixed subsidy at the 60% rate. In Tbilisi there is no fixed subsidy; Georgia is outside the EU. The fixed prices include CBCT, implants, surgery, final restoration, transfers and coordinator; we allow €1,500 for flights and hotel.

Treatment Price in Germany Fixed subsidy 60% Your share in Germany Tbilisi (no subsidy) Tbilisi incl. travel
Single implant + zirconia crown €3,000 €552.96 €2,447 from €629 (£540 / $730, NTA®) / €1,258 (£1,080 / $1,450, Straumann®) approx. €1,600 / €2,250
All-on-4, one jaw €15,000 €576.44 €14,424 from €3,890 (£3,340 / $4,500) approx. €5,390
All-on-4, both jaws €30,000 €1,194.16 €28,806 from €7,780 (£6,680 / $9,000) approx. €9,280

To be honest: for a single implant the trip rarely pays off. With Straumann® and a ten-day stay you end up at around €2,250, in Germany at €2,300 to €2,450. It is different from two implants upwards and for the edentulous jaw. Run your own case through the implant cost calculator; all prices are in the price list.

5. Hardship rule: who gets the double fixed subsidy

Those on a low income get the standard treatment paid in full. Under § 55 (2) SGB V, a further 40% is added to the 60% subsidy. For 2026 the limit is €1,582.00 gross monthly income for a single person, €2,175.25 with one dependant and €395.50 more for each further dependant, according to the Verbraucherzentrale (as of 27 January 2026).

Without an income check, the hardship rule applies to recipients of Bürgergeld (the basic citizen’s allowance), basic income support, social assistance and BAföG student grants. You have to apply for it at your insurer, ideally together with the treatment and cost plan.

Those just above the limit get part of it under § 55 (3): the insurer reimburses the amount by which the 60% subsidy exceeds three times the difference between income and limit. Example: €100 over the limit, subsidy €552.96, the insurer pays an additional €252.96. The hardship subsidy also applies to a different type of treatment, so for a single implant €921.60 instead of €552.96.

6. Dental top-up insurance for implants: what 80–100% really means

A good dental top-up insurance policy can indeed cover 80 to 100% of the bill for an implant, but only if the contract was in place years before the first diagnosis. Anyone who has a gap today and buys a policy tomorrow gets nothing for that gap.

The Verbraucherzentrale (as of 23 July 2026) names the traps: often an eight-month waiting period, then a benefit cap (Zahnstaffel) in the first three to five years, for example €500 in the first year and €1,000 in the first two years combined. Treatments already recommended are excluded.

Look for: no cap on implant benefits, bone grafting covered, reimbursement up to 3.5 times the GOZ rate. With most providers the percentage refers to the total bill including the fixed subsidy. Stiftung Warentest, the German consumer testing foundation, reviews the tariffs regularly. The contract makes sense for younger people with healthy teeth, not any more once the diagnosis is there.

7. Check-up booklet: 10 years for 15% more

For a single implant, the check-up booklet brings €92 more fixed subsidy after five years and €138 after ten; for two edentulous jaws up to €298. Not a fortune, but free.

The rule under § 55 (1) SGB V: you get 70% if you have attended a check-up every year in the last five calendar years (children every six months), 75% after ten calendar years without interruption. The insurer may forgive a single gap in the ten years in justified cases, but does not have to. Check the booklet before the treatment and cost plan is submitted.

8. Treatment and cost plan: how to read it before you sign

The treatment and cost plan (Heil- und Kostenplan, HKP) is the most important page of your treatment. Without an approved HKP the insurer pays no fixed subsidy, not even retrospectively. It must be submitted and approved before treatment begins.

Part 1 is the insurer’s part: diagnosis per tooth, standard treatment, planned treatment, fixed subsidy codes such as 2.1, bonus field. Part 2 is the private itemisation under the GOZ with its multiplier. Check: is the bonus correct? Is a GOZ multiplier above 2.3 justified? Are bone grafting, temporary and sedation listed? From €3,000 a second opinion is worthwhile; the patient advice centres of the dental chambers check the HKP free of charge.

9. What the insurer never pays: bone grafting, sinus lift, temporary, sedation

Everything that makes the implant possible in the first place is a private service without subsidy. Bone grafting costs around €300 to €1,500 per area, an external sinus lift €600 to €1,200. These items belong in the HKP, not only on the final bill.

The ranges according to the dental implant cost calculator (updated 5 June 2026) and a cost breakdown from December 2024: bone substitute material €60 to €300, internal sinus lift €300 to €550, external sinus lift €600 to €1,200, bone splitting €1,000 to €1,500. In Tbilisi these items are in the written quote before you travel, with no extra charges at the chair.

10. Implant or bridge: when the standard treatment is the right choice

For a single gap a bridge is often sensible, and the insurer pays the same fixed subsidy. If the bridge costs €1,200 to €2,000, your own share is €650 to €1,450 instead of €2,300 to €2,450 for the implant.

The price of the bridge is not only in euros: the neighbouring teeth are ground down. If they are already crowned you lose little; if they are healthy you sacrifice tooth substance. An implant is usually the better choice with healthy neighbouring teeth, with a free-end gap or for the whole jaw. More under dental implants in Tbilisi.

11. Instalments and financing

Almost every practice offers payment by instalments through a billing company, short terms often interest-free, longer ones with interest. Check the effective annual rate; for terms of several years it is often higher than a personal loan from your own bank.

Taking out dental top-up insurance after the diagnosis does nothing for this treatment. Your own share counts as an extraordinary burden for tax purposes. In Tbilisi you pay according to a written fixed price; we do not offer instalments.

12. Abroad: EU yes, Georgia no, and why it is still cheaper

Under § 13 (4) SGB V the fixed subsidy is only available for treatment in the EU, the EEA and Switzerland. In Georgia or Turkey the insurer pays nothing. Even so, Tbilisi remains more than €9,000 cheaper for an edentulous jaw.

Within the EU the HKP must be approved in advance; you pay yourself and get the fixed subsidy reimbursed, according to the Verbraucherzentrale (as of 23 February 2026). The steps are on our page Festzuschuss for dental prosthetics abroad (in German).

All-on-4, one jaw Price Fixed subsidy Travel What you pay
Germany €15,000 €576.44 €0 approx. €14,424
Another EU country (example €8,000) €8,000 €576.44 approx. €1,000 approx. €8,424
Tbilisi, fixed price €3,890 (£3,340 / $4,500) €0 approx. €1,500 approx. €5,390

For a single implant, Germany or an EU neighbour with subsidy often remains the better choice. Abroad, brand, warranty and aftercare matter: our partner clinics work with Straumann® and Bredent®, and you receive an implant passport. Clarify in writing before you travel who pays if a correction is needed.

Frequently asked questions

Does German health insurance pay for a dental implant?

No, not the implant itself. The statutory insurer pays the fixed subsidy for the standard treatment, meaning a bridge or denture, regardless of whether you choose an implant. Only in rare exceptions such as the consequences of a tumour, an accident or genetic absence of teeth does it cover the implant and abutment in full.

How much is the fixed subsidy for an implant in 2026?

For a single missing tooth €552.96 without bonus, €645.12 after five and €691.20 after ten years of check-up booklet. For an edentulous upper jaw €576.44 to €720.56, for the lower jaw €617.72 to €772.16. Under the hardship rule 100% of the standard treatment. From 2027 the rates fall to 50, 60 and 65%.

What does AOK pay for a dental implant?

Exactly as much as TK, Barmer or DAK: the statutory fixed subsidy, so in 2026 €552.96 for a single missing tooth without bonus. AOK may not pay more than other insurers. Differences exist only in bonus programmes and brokered dental top-up insurance, which are private contracts with a waiting period.

Is dental top-up insurance worth it for implants?

Only if you take out the contract years before the first diagnosis. Recommended treatments are excluded, and there is usually an eight-month waiting period and a benefit cap of around €500 in the first year. Look for tariffs with no cap on implants, with bone grafting and up to 3.5 times the GOZ rate.

Who gets the double fixed subsidy (hardship rule)?

Single people with gross income up to €1,582.00 a month (2026), with one dependant up to €2,175.25, €395.50 more for each further dependant. Also recipients of Bürgergeld, basic income support, social assistance or BAföG. Those just above the limit receive part of it under the sliding rule. The hardship rule must be applied for and also applies to implants.

Is the fixed subsidy also available for treatment abroad?

Only in the EU, the EEA and Switzerland (§ 13 (4) SGB V), and only with a treatment and cost plan approved in advance. For Georgia or Turkey the insurer pays nothing. For an edentulous jaw Tbilisi still remains more than €9,000 cheaper; for a single implant the trip is usually not worth it.

The short version

  • The insurer never pays for the implant, only the fixed subsidy for a bridge or denture: in 2026 €552.96 to €691.20 for a single missing tooth, €921.60 under the hardship rule. AOK, TK and Barmer pay identically.
  • Dental top-up insurance only helps if it has been in place for years before the diagnosis. The treatment and cost plan must be approved before treatment.
  • Abroad, the subsidy is only available in the EU, EEA and Switzerland. A single implant rarely justifies the trip; All-on-4 costs from €3,890 (£3,340 / $4,500) in Tbilisi compared with around €14,400 own share in Germany.
